Teaching Programs

The Course begins on july 1st
Completed application forms should be sent along with the registration fee of rs.150/- (One hundred and fifty only) drawn in favour of "CSI HOSPITAL, BANGALORE" along with copies of certificates, so as to reach the NURSING DIRECTOR, School of Nursing, CSI Hospital, Bangalore - 51, on or before 15th June 2007. Incomplete forms will be rejected. Do not enclose original certificates.
The candidate's application number must be written on all enclosures and correspondence.
Declaration must bear the signature of student and his/her parent/guardian.
The following certificates have to be enclosed:

Two sets of SSLC and Plus 2 certificates photocopy (attested by a gazetted officer). The candidate should submit certificate of verification of the authenticity and genuineness of the mark certificates from the examination board.
Copies of certificates of other educational qualifications, if any.
Transfer certificate from School/college last attended.
Two certificates of conduct - One from the head of the High School where studied and the second from a Pastor. If non-christian, from a reputable citizen.
Medical Fitness certificate from a regiastered medical practitioner.
Two recent passport size photographs.
A short essay.
Stamped self-addressed envelope 9" x 4"

All information furnished in the application must be correct. Any false information or suppression of any factual information would lead to disqualification. If the fact that any factual information has been suppressed or any false information given comes to notice, the candidate is liable to be terminated besides other actions by the institution as may be deemed by it.
